Conference Rate Details
Academic Full Day Only will provide you with access to the academic session taking place at The Hart House, University of Toronto Campus. This will be a full day event from 8:30am – 4:30pm. Onsite registration may be available if space permits.
Academic Full Day & Gala Dinner will provide you with access to the academic session taking place at The Hart House, University of Toronto Campus. It will also provide you with one admission to the Gala Evening Reception & Dinner at Arcadian Loft located in the Simpson Tower at 401 Bay Street in downtown Toronto. Doors will open for the reception and dinner at 6:30pm.

Registration Policies & Information
Please note that once you’ve registered, you will be issued a confirmation email and receipt. This can be used for reimbursement as required. Registration receipts will not be issued on site. We do not issue letters of invitation for registered delegates. Online registration will close on Tuesday, November 5th at 5:00pm. Onsite registration may be available if space permits.
Payment by VISA, MasterCard or cheque must accompany the completed online registration form. All cheques must be made payable to the order of University Health Network. Payment must be received on or before Monday, October 28th, 2013.
A full refund, minus a $75.00 administration fee, will be issued for cancellations receive via e-mail on or before Monday, October 28th, 2013. No refunds will be issued after this date. Refunds will not be granted to registrants who do not attend the conference.
