Industry: $500.00

CANCELLATION POLICY
A full refund, minus a $75.00 administration fee, will be issued for cancellations received via facsimile (416-597-6202) or email (conferences@uhn.ca) on or before January 5, 2015. No refunds will be issued after this date. Refunds will not be granted to attendees who do not attend the conference.

PAYMENT
Payment by Visa, MasterCard or cheque must accompany the completed online registration form. Payment must be received on or before January 5, 2015 for the conference.
Registration deadlines and fees are subject to change.
Please note we do not issue letters of invitation for registered delegates.
